USE FIRST AID

Notify supervisor to report injury

Fill out Incident Reporting Form

Print and fax to EHS at (870) 972-3584

Transport employee to closest ER by ambulance or State vehicle

Stay with employee until family arrives

Supervisor must call the Company Nurse Injury Hotline at 1-855-339-1893 to report injury

Employee and supervisor must come by EHS to review and sign WC forms within 48 hours

Deliver all additional paperwork to EHS

Prescriptions are to be filled using prescription form

Refills are at the discretion of the attending physician

Prescription cost will be billed directly to A-State
