ROTC Cadet Forms

Welcome to the A-State ROTC Forms Page! This site is populated with forms that Cadets will need to fill out at each stage of Cadet enrollment. The following are some notes to help to ensure you can successfully open, fill out and save these interactive documents. 

1. Ensure that you have at a minimum Adobe DC Reader. This should be a free download. Do not purchase this item as it is available free from adobe.
2. Ensure that you save each form down onto your computer as this will allow each computer to access the form.
3. Do not digitally sign your document, you will need to ink sign all forms. 
4. Save completed documents to a computer or device you can secure. Bring the device with you on your next visit.

ADVANCED COURSE or CONTRACTING FORMS

These forms are to be filled out after you have completed and submitted the basic course forms. These are not required for initial enrollment.

• SGLV 8286 (PDF_MAR 2023) - Soldier's Group Life Insurance Form
• DA Form 4824 (PDF) - Simultaneous membership program agreement for Army Reservists. This must be submitted to us if you elect to do the SMP program with the USAR. Not required for those planning on becoming Active Duty Army Officers.
• NGB Form 594-1R (PDF) - Simultaneous membership program agreement for Army National Guard Cadets. This must be submitted to us if you elect to do the SMP program with the National Guard. Not required for those planning on becoming Active Duty Army Officers.
